Prisma Health Care Collaborative

PRISMA is the word for “prism” in Spanish, Italian and many other languages. We chose this symbol of transformation and change as our logo to represent how we think about healthcare.

 

There are many aspects of health and wellness, and as your healthcare provider, we need to understand these and how they fit together. We know that the patient we see in front of us is a complex interplay of genetics, environment, life experiences, habits and much more! We need to pay attention to all of these things to help you achieve your health and wellness goals.

 

As a group of providers, we also have many different areas of interest and expertise. Our goal is to work together, as a collective, to serve you and your family in the best way we can.
